Booster Club Check Request Form Download This form is to be used by any parent or FTE Staff who needs to be reimbursed by Booster Club for items purchased. Related receipts should be attached to the form. The form should then be placed in the Booster Club Treasurer folder in the front office.
Booster Club Tax Free Form Download If making purchases on behalf of Booster Club, please present a copy of this form to the vender to validate our tax-exempt status. This will allow us to avoid sales taxes on Booster Club related purchases.
